We offer hope and tools to help our clients manage conflict more effectively, communicate authentically, heal from infidelity, rebuild trust after betrayal, deepen both emotional intimacy and physical intimacy.
We understand the importance of creating a welcoming environment, backed by sound theory, research and practice that helps our clients to feel safe and understood.
He is calm, quiet & empathetic. My husband and I have been seeing him for almost 6 months and we’re on such an amazing path. He offers real solutions and real advice. He is extremely professional. The best part is he is so non-judgmental and really sees past the “situation” and links it into the behaviour and the intended outcome via the real outcome. I am grateful we found him. Trust me, we’re not fixed and need to continue our therapy. I would highly recommend Dr. Nathan Cobb. He is a VERY safe place.

view profileOur current system allows you to complete the New Client Intake Form online. The PDF version on this page is only for situations where a new client at the Calgary Couples Counselling Centre is unable to complete the forms online. In such a case, you can download and print the form below (8 pages), fill it in, and bring it with you to your first appointment (if you are attending in-person).
There are several sections of the New Client Intake Form that we ask each new client to complete:
Use this link instead of the above link, to complete the New Client Intake Form, if the counselling is for a child or youth under the age of 18, or if you are seeking family therapy and plan to have children or youth present in the sessions (there are additional consent forms to complete for child or family therapy). You do not need to complete this additional form if you are not seeking therapy for your child or family therapy with your children.
If you will be engaging in video counselling, we have an additional Informed Consent form that needs to be signed. For clients who complete our intake forms online, the e-therapy informed consent is built in to the online form. If you are unable to complete the intake forms online, however, and prefer to print them off as a PDF and your therapy sessions will be conducted by video, please download, print and sign the additional E-Therapy Informed Consent form using the link below.
